My Health Online is an online service brought to you from NHS Wales. Depending on your practice you may be able to order repeat prescriptions online or change your contact details.

We now need you to provide proof of your identification and your address. We ask for 3 things:

Step 1

A photo of your passport or driving licence, as proof of your name;

Step 2

A photo of a recent utility bill or similar, from sometime over the past 3 months, as proof of your current address;

Step 3

A short “selfie video”, in which you give us your name and ask us to register the account for you (see below for exactly what we ask you to say).

BSL signed or written on a piece of paper and displayed is an acceptable form of registration

Please note that we use these items once for verification when setting up your account, and we delete them as soon as your account is open.

Please take and upload a short video of yourself saying: “My name is ... and I would like access to the online services as requested in the form”. BSL signed or written on a piece of paper and displayed is an acceptable form of registration
