If you change name, address, email or telephone number, please let our Patient Assistants know as soon as possible by completing the change of details form which is available to download below (or from reception).
Please then bring the form to reception, with proof of the changes (marriage certificate etc).
If you move outside the practice area you may need to find a doctor in your new area. There are instructions on what to do when you move to a new area on the back of your NHS medical card.
Are you expecting an appointment in a clinic or hospital? Please inform the clinic or hospital of your new details too.
